Welcome to Care Resources
Care Resources is a Program of All-inclusive Care for the Elderly (PACE®). Care Resources was developed as a health care program for older adults needing long-term care and wishing to age in place. As we age, medical problems become more complex and last longer. Care Resources is responsible for all primary and specialty care for those who are enrolled.

PACE is different from any long-term care option. We partner with you to stay independent and in good health, as you age. We have health care professionals who watch for changes in your condition, provide treatment, and encourage you to stay independent. When you enroll in the program, you will receive services unique to your own needs. The PACE model is an all-inclusive health care program that promotes better care and better health.
What are PACE Services?
PACE is a national model of care that has been developed to provide necessary services to each participant. PACE is responsible to provide both Medicaid and Medicare (Part A, Part B, and Part D) services.
The PACE benefits may include but are not limited to:
- Primary and Specialist Care (including Women's Health)
- Medical Transportation
- Therapy (Occupational, Physical, Speech)
- Recreational Day Center Activities
- Home Care
- Hospital & Emergency Services
- Foot Care
- Nursing Home
- End of Life Supportive Care
Not all people in PACE receive all these services. Services are based on the recommendation of the Interdisciplinary Team (IDT). Once enrolled in the program, participants must lock-in to the PACE network of providers or be liable for out of pocket costs (except emergency care).
